We are seeking a highly skilled and experienced Pediatric Care Coordinator to join our team at Children's Healthcare of Atlanta. As a key member of our nursing team, you will play a critical role in ensuring the seamless coordination of patient care across our facilities.
Key Responsibilities- Develop and implement effective care plans to optimize patient outcomes and flow
- Collaborate with interdisciplinary teams to ensure continuity of care and address any care coordination issues
- Provide education and support to patients, families, and caregivers to promote health and wellness
- Utilize advanced clinical assessment skills to identify and address patient needs
- Maintain accurate and up-to-date records of patient care and outcomes
- 5 years of experience in nursing with 2 years of experience in critical care or emergency nursing
- Advanced clinical assessment skills and ability to integrate pathophysiology, psychology, and sociology into the pediatric clinical setting
- Experience in progressive leadership and training as house supervisor or bed placement coordinator
- Experience in psychiatric or behavioral health and medical inpatient/emergency department settings
- Graduation from an accredited school of nursing and certification in Case Management or Psychiatric Mental Health
