Key Responsibilities

The Home Health Patient Care Manager is accountable for overseeing and coordinating clinical services, delivering nursing care to patients in their homes as directed by physicians, and adhering to relevant laws, regulations, and agency policies.

Your responsibilities will include:

  • Providing clinical services within the defined scope of nursing practice, in alignment with care plans and in collaboration with the healthcare team.
  • Managing referrals, assigning appropriate clinicians, evaluating patient orders, and scheduling initial care visits.
  • Coordinating patient home health benefits, assessing medical necessity, and securing ongoing insurance approvals.
  • Continuously assessing patient needs and ensuring that care is tailored to individual requirements, meets eligibility criteria, and complies with physician directives.
  • Reviewing assessments and care plans daily, consulting with clinicians for recommendations as necessary.
Qualifications

To be considered for this role, candidates must possess:

  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • A valid Driver's License, vehicle insurance, and access to reliable transportation.
