Job Description:

The Medical Team Coordinator role is a key position on our team at Henry Ford Health St. John Hospital. As a Registered Nurse, you will be responsible for coordinating patient care, communicating effectively with the healthcare team, and ensuring seamless transitions between departments.

Responsibilities:

  • Coordinate patient care and communicate effectively with the healthcare team.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Serve as a liaison between departments to ensure smooth transitions and quality care.

Requirements:

  • Diploma from an accredited school of nursing or required professional licensure at time of hire.
  • BLS Provider certification within 1 month of hire date.
  • Registered Nurse credentialed from the Michigan Board of Nursing.

Compensation and Benefits:

  • Estimated Salary: $85,000 - $115,000 per year.
