About the Role

We are seeking a skilled Care Coordination Specialist to join our team at the Detroit Medical Center. As a key member of our care coordination team, you will play a critical role in ensuring seamless transitions for our patients.

The ideal candidate will have a Master's degree in Social Work and at least three years of experience in a hospital setting. Previous supervisory experience is strongly preferred. You will be responsible for coordinating care services for patients, including arranging transportation, scheduling appointments, and communicating with healthcare providers.

You will also be responsible for identifying and addressing potential barriers to care, such as lack of insurance or access to transportation. Your duties will include providing education to patients and their families on available resources and benefits, as well as participating in quality improvement initiatives.

We offer a competitive salary range of $80,000 - $105,000 per year, depending on experience, plus a comprehensive benefits package.
