Position Overview:
The role involves coordinating, supporting, and assisting families and children in accessing essential services from the Sunshine Health Plan. The Nurse Care Coordinator will manage an active caseload of children and youth in out-of-home care, ensuring timely and updated assessments. It is crucial to adhere to designated timeframes for the treatment of children under the Sunshine Child Welfare Specialty Plan. The position also includes providing a case management overlay for medical treatment and evaluating medical records to determine the appropriate level of care and discharge planning.

Additionally, the Nurse Care Coordinator will advocate for clients, offering training and information regarding medical necessity requirements to clinical staff and subcontracted Case Management and Child Placing Agencies, ensuring compliance with the highest standards and mandated requirements.


Key Responsibilities:

Caseload Management:
Manage an active caseload of children and youth in out-of-home care, ensuring timely and updated assessments.

Care Coordination:
Ensure compliance with designated timeframes for treatment and provide case management overlay for medical services.

Records Assessment:
Evaluate records to confirm the appropriate level of care and treatment, including discharge planning verification.

Assessment and Support:
Identify and refer individuals who may require physical or behavioral healthcare management, health coaching, or care coordination to the Sunshine Health Plan.

Advocacy and Training:
Deliver client advocacy, training, and information about medical necessity requirements to clinical staff and subcontracted agencies, ensuring adherence to high standards.

Effective Communication:
Exhibit strong verbal communication skills with children, caregivers, stakeholders, and staff.

Team Collaboration:
Participate in Family Team Conferencing as needed to address client needs.

Additional Duties:
Fulfill other incidental or related duties as assigned, assist and train others, and perform responsibilities of higher-rated positions for developmental purposes.

Qualifications:

Registered Nurse (RN) with a minimum of 3 years of experience, preferably in child welfare or behavioral/medical healthcare settings. Strong organizational and time-management skills are essential, along with proficiency in data analysis and Microsoft Office Suite.


Experience Requirements:
RN license required. MA/MS preferred. 3+ years of relevant experience required, with experience working with DCF and/or ACHA preferred.

Physical Requirements:

Ability to lift and carry 25 pounds, with varying percentages of reaching, sitting, stooping, crouching, and bending.
