Position Overview:
The role involves coordinating, supporting, and assisting families and children in accessing essential services through the Sunshine Health Plan. The Nurse Care Coordinator will manage an active caseload of children and youth in out-of-home placements, ensuring timely and updated assessments. The position requires adherence to specified timeframes for the treatment of children under the Sunshine Child Welfare Specialty Plan, along with providing a case management overlay for medical treatment.
Key responsibilities include reviewing medical records to determine the appropriate level of care and treatment, including discharge planning. The Nurse Care Coordinator will also advocate for clients, offering training and information regarding medical necessity requirements to clinical staff and subcontracted agencies, ensuring compliance with the highest standards and mandated requirements.
Technical/Functional Responsibilities:
Case Management-
Core Responsibility:
Oversee an active caseload of children and youth in out-of-home care, ensuring timely and updated assessments.
Care Coordination-
Core Responsibility:
Ensure compliance with designated timeframes for treatment of children covered by the Sunshine plan, providing case management support for medical treatment.
Records Assessment-
Core Responsibility:
Evaluate records to confirm the appropriate level of care and treatment, including discharge planning verification.
Assessments and Referrals-
Core Responsibility:
Identify and refer individuals who may require physical or behavioral healthcare management, health coaching, or care coordination to the Sunshine Health Plan.
Advocacy and Training-
Core Responsibility:
Provide advocacy for clients, along with training and information on medical necessity requirements to clinical staff and subcontracted agencies, ensuring compliance with the highest standards.
Effective Communication-
Core Responsibility:
Demonstrate excellent verbal communication skills with children, caregivers, stakeholders, and staff. Participate in multidisciplinary team meetings as needed to address client needs.
Additional Duties:
The role may require fulfilling other incidental or related duties as assigned, assisting and training others, and occasionally performing tasks of higher-rated positions for developmental purposes.

Qualifications:
Registered Nurse (RN) with a minimum of 3 years of nursing experience, preferably in child welfare or behavioral/medical healthcare settings. Strong organizational and time-management skills are essential for managing multiple tasks effectively. Proficiency in data analysis and Microsoft Office Suite (Word, Outlook, Excel) is required.
Experience Requirements:
RN license is mandatory. A Master's Degree or equivalent is preferred, along with 3+ years of relevant experience. Experience working with DCF and/or ACHA is advantageous.
Physical Requirements:
Ability to lift and carry up to 25 pounds, with varying requirements for reaching, sitting, stooping, crouching, and bending.
