Job Summary:

We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Health First. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and promoting a positive patient experience.

Key Responsibilities:

  • Quality and Safety: Ensure that patient care is delivered in accordance with professional nursing standards and best practices. This includes:
  • Effective Medication Administration: Demonstrate expertise in medication administration, as evidenced by preceptor observation, skills fair demonstration, and clinical practice.
  • Leadership and Delegation: Demonstrate effective leadership and delegation skills, ensuring that patient care activities are supervised and coordinated within the healthcare setting.
  • Communication and Teamwork: Foster effective communication and collaboration with other healthcare team members, promoting a positive and supportive team environment.
  • Health Information Technology: Utilize Health First's computer systems for order entry, order inquiry, and patient care documentation.
  • Clinical Practice and Competency: Demonstrate safe and effective clinical practice, as evidenced by skills fair demonstration, yearly competency assessment, and age-specific checklist.
  • Interdisciplinary Care Planning: Coordinate and implement interdisciplinary plans of care based on current evidence-based practice.
  • Documentation and Record-Keeping: Accurately document all nursing care, changes in condition, response to therapy, treatments, procedures, and referrals on patient records.
  • Dependability and Flexibility: Demonstrate dependability, reliability, and flexibility in meeting the needs of patients and the healthcare team.

Customer Experience:

  • Patient Education: Provide patient, family, and/or caregiver education as directed by the plan of care.
  • Cultural Sensitivity and Respect: Deliver patient care with sensitivity and respect for individual differences, including age, culture, and personal preferences.
  • Positive Patient Experience: Contribute to a positive patient experience, as evidenced by positive patient satisfaction scores and HCAHPs scores for the department/medical center.

  • Resource Management: Utilize equipment and supplies economically, ensuring efficient use of resources.
