**Job Title:** Veterans Support Coordinator

New Directions for Veterans (NDVets) is dedicated to improving the lives of veterans through our innovative recovery programs. We are seeking a highly motivated and compassionate Veterans Support Coordinator to join our team.

**Job Summary:

  • Coordinates care and services for veterans, ensuring they receive comprehensive support and guidance.
  • Develops and implements individualized treatment plans, working closely with clients to set and achieve goals.
  • Provides ongoing support, crisis intervention, and relapse prevention through counseling, groups, and seminars.

**Key Responsibilities:**

  • Conduct pre-placement screenings to assess housing needs and potential for retention.
  • Collaborate with treatment teams, including supervisors, case managers, peer counselors, and psychiatric consultants, to provide coordinated care.
  • Maintain accurate and up-to-date records, completing all required documentation within specified timeframes.
  • Ensure data quality, accuracy, and completeness, tracking client progress and outcomes.

**Required Skills and Qualifications:

  • Bachelor's degree in a related field or equivalent experience.
  • Possess excellent organizational skills, time management abilities, and initiative.
  • Demonstrate proficiency in basic office software, including Microsoft Office and HMIS.
