  • Stay informed about benefits and services available to clients.
  • Conduct thorough assessments of clients' housing needs, particularly concerning their ability to maintain independence in the community.
  • Work collaboratively with clients to develop service plans and review them periodically for necessary adjustments.
  • Ensure all client data and service details are accurate and up-to-date while adhering to confidentiality and HIPAA regulations.
  • Complete all required documentation for client files in accordance with agency policies.
  • Support clients in developing self-advocacy skills and independent living capabilities through active participation in achieving service goals.
  • Act as a liaison and advocate with other service providers on behalf of clients.
  • Contribute to the organization's advocacy efforts by reporting observed barriers and successes related to benefits and services for individuals with disabilities, participating in advocacy activities, and identifying clients who represent specific advocacy issues.
  • Engage in community outreach, presentations, and workshops as assigned.
  • Attend in-service training, unit meetings, and other agency-related activities.
  • Perform additional program-related duties as required.

Experience:

  • Respectful and comfortable working with individuals from diverse backgrounds.
  • Understanding or experience with disability-related issues is preferred.
  • Multilingual abilities are a plus.
  • Comprehensive knowledge of housing development, the public and private real estate sectors, and familiarity with Public Housing Authorities.
  • In-depth understanding of state and federal housing programs, awareness of housing issues affecting individuals with disabilities, and experience in fostering productive partnerships both internally and externally.
