Role Overview: The Senior CCBHC Case Manager is responsible for delivering comprehensive case management and crisis intervention services. This role is essential in facilitating the coordination of treatment services aligned with workforce initiatives, while enhancing access to transitional support for clients facing various challenges.
Key Responsibilities:
- Conduct support group sessions focusing on essential life skills.
- Act as an advocate for clients to secure necessary services.
- Engage with relevant agencies and systems (such as DSS, probation, and courts) to support clients in fulfilling all requirements.
- Provide ongoing case management addressing needs related to housing, transportation, childcare, nutrition, and other essential services.
- Administer toxicology tests when required.
- Maintain effective communication with direct service and vocational providers.
- Prepare weekly schedules and optimize time management.
- Participate in staff and community meetings as necessary.
- Deliver crisis intervention and group services, providing backup coverage when needed.
- Utilize appropriate computer skills to ensure effective job performance.
- Uphold quality care standards in accordance with the agency's Medicaid Compliance Plan and accountability systems.
- Report any compliance violations through the designated supervisory channels.
