Job Title: Adult Referral Navigator

Location: HOPE Clinic

Reports to: Adult Referral Navigator Team Lead

Job Summary: We are seeking a skilled Adult Referral Navigator to coordinate patient referrals for the Asian American Health Coalition (AAHC) DBA HOPE Clinic.

Key Responsibilities:

  • Provide detailed referral information to patients
  • Organize follow-up activities for patient visits
  • Stay updated on external referral sources
  • Schedule appointments and referrals
  • Follow-up on appointments and results
  • Initiate pre-authorization for procedures
  • Verify patient insurance and information
  • Respond to customer and patient questions
  • Document all patient information securely
  • Submit Agency forms and documentation
  • Contribute to staff morale and corporate culture
  • Maintain HIPAA standards of privacy
  • Attend community engagement activities
  • Perform other duties as assigned

Requirements:

  • Minimum one year experience in coordinating referrals
  • Medical and insurance experience preferred
  • Strong communication and customer service skills
  • Detail-oriented and able to handle multiple tasks
  • Basic knowledge of Microsoft Office and Internet

Education and Experience: High School diploma or GED required; Associates Degree preferred. Experience in the medical field and referral/health insurance is preferred.

Language Skills: Fluently bilingual in another language served by HOPE Clinic and English.
