Location: Lebanon, New Hampshire

Overview:
The Director of Care Management is accountable for overseeing comprehensive Care Management functions, including Capacity Coordination, Social Work, Navigation, and Case Management. This role involves executing the strategic plan for care management across the health system and health plan.

Key Responsibilities:
- Lead the development of tactics in collaboration with the VP of Care Management and Hospital Chief Nursing Officers, ensuring alignment with performance metrics and the vision for the care management program, which includes:
- Adherence to all regulatory requirements
- Ensuring timely progression of patient care
- Optimizing care and resource utilization
- Utilizing data analytics to enhance system performance in care management

- Work alongside physicians and departmental leaders to promptly address barriers impacting care management outcomes.

- Analyze and disseminate Care Management data to Dartmouth Health executives and departmental leaders as necessary.

- Manage personnel development, including mentoring and coaching of Care Management leaders and staff.

- Collaborate with Care Management and Dartmouth Health system leaders to establish the operational framework for the department.

- Foster engagement with Care Management leaders across the system to create and implement standardized roles and staffing models.

- Develop and maintain educational and resource materials for Care Management.

- Establish professional standards for care management in accordance with recognized care standards, regulatory guidelines, and system policies.

- Cultivate a culture of trust and engagement, as reflected in employee feedback surveys.

- Draft and propose Care Management policies and procedures for final approval.

- Create comprehensive guidelines and processes for managing complex patient cases.

- Monitor compliance with regulatory standards, ensuring adherence to conditions of participation.

- Represent Dartmouth Health Care Management at various meetings and through educational initiatives.

Qualifications:
- Master's degree in a clinical or health-related field is required.
- A minimum of five to seven years of leadership experience in care management, case management, or utilization review within an integrated healthcare system or health plan.
- Preferred certifications include Certified Professional in Healthcare Quality (CPHQ), Certified Case Manager (CCM), Certified Professional Utilization Management (CPUM), or similar.
- Strong leadership, communication, and technological skills are essential.
- Extensive knowledge of payer systems and clinical utilization management is required.
- Familiarity with clinical systems, outcome measures, and patient information systems is necessary.
- Must possess analytical skills for data interpretation and be driven by data in medical management.
- A valid Registered Nurse license in New Hampshire is required.
- Certification as an Accredited Case Manager or similar is mandatory.
