The Patient Care Coordination Associate, under the supervision of the Rehabilitation Services Manager, delivers comprehensive clerical, telephonic, and administrative support to facilitate patient care coordination within the acute care environment and in collaboration with community and discharge settings. Key tasks may involve making referrals to community resources and coordinating transportation services. Responsibilities include, but are not limited to: transmitting clinical information for post-discharge placements or equipment needs, facilitating admissions to designated skilled nursing facilities, scheduling follow-up appointments with primary care physicians, and delivering important Medicare messages to eligible patients. Administrative duties encompass typing, transcription, file management, departmental scheduling, supply ordering, and preparing documents for essential departmental committees.

Key Responsibilities

  • Acts as the primary liaison between care management teams and post-discharge facilities or agencies.
  • Sends clinical information for placements at post-discharge facilities such as skilled nursing facilities, intermediate care facilities, and home health services.
  • Coordinates post-discharge Durable Medical Equipment (DME) needs.
  • Schedules follow-up appointments with primary care physicians and completes necessary documentation.
  • Establishes primary care providers for patients lacking one.
  • Organizes transportation for discharged patients, notifying staff of discharge times and ensuring proper documentation in medical records.
  • Arranges ongoing transportation for dialysis patients.
  • Verifies Medicaid eligibility and communicates findings to relevant staff.
  • When directed by the Care Management Team, delivers important Medicare messages.
  • Responsible for documenting Utilization Review communications and conveying information to the Utilization Management RN.
  • Demonstrates strong interpersonal skills in interactions with all stakeholders and escalates concerns to management as needed.
  • Contributes to departmental growth by supporting and participating in the development and execution of departmental goals and initiatives.
  • Prioritizes tasks effectively and completes them in a timely manner.
  • Identifies innovative solutions and takes proactive measures.
  • Utilizes critical thinking and problem-solving skills efficiently and seeks guidance when necessary.
  • Serves as the timekeeper for the department.
  • Manages scheduling for departmental staffing and meetings.
  • Attends departmental meetings and produces accurate and concise minutes.
  • Answers departmental phone lines and directs calls appropriately.
  • Provides typing and transcription support for the Manager as required.
  • Maintains and organizes departmental files.

Qualifications

Education:
A bachelor's degree is preferred, or equivalent experience.

Experience:
A minimum of two years of experience in the healthcare sector is required. Experience in administrative support roles is preferred. Proficiency in Access, Excel, Word, and standard office software is essential. Familiarity with continuous quality improvement, outcomes management, customer satisfaction, and regulatory standards for information management is strongly preferred.

Knowledge and Skills:
Understanding of the utilization review process is preferred. Knowledge of available data systems within the organization, particularly those relevant to the assigned department, is advantageous. Strong analytical and customer support skills are necessary. Ability to manage conflicts, prioritize competing demands, and problem-solve effectively is essential. Excellent verbal and written communication skills are required. A background in medical terminology is preferred. Demonstrated proficiency in word processing and database management is expected. Professional behavior and the ability to maintain confidentiality in the workplace are crucial.
