Pegasus Senior Living is seeking a dedicated and compassionate Health and Wellness Coordinator to join our team. As a key member of our care team, you will be responsible for ensuring the highest level of care and services for our residents.

Responsibilities
  • Assure implementation of policies and procedures relating to Resident care.
  • Supervise and assist in provision of all Resident services, including coordination of services provided by independently contracted ancillary health care providers.
  • Conduct assessments, negotiate service plans, develop service schedules, and update service information as indicated by Resident needs.
  • Ensure Residents are appropriately assessed on a timely basis and charged in accordance with those assessments.
  • Assist in Social History Assessments and outline life enrichment programming through these assessments.
Requirements
  • Must be an LVN or LPN.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of healthcare policies and procedures.
