Join Our Team as a MDS RN or LPN Coordinator

At Ark healthcare & Rehabilitation at St. Camillus Center, we are seeking a highly skilled and detail-oriented MDS RN or LPN Coordinator to join our team. As a key member of our interdisciplinary team, you will play a crucial role in ensuring the accurate and timely completion of the resident assessment instrument: MDS, CATs, and CAAs.

Key Responsibilities:

  • Contribute to the facility's interdisciplinary RAI process while following RAI guidelines for completion, computer coding, and electronic transmissions of the MDS.
  • Coordinate the completion of the resident assessment instrument: MDS, CATs, and CAAs, in accordance with current rules, regulations, and guidelines that govern the resident assessment.
  • Manage the overall process and tracking of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for services provided within the Center.
  • Conduct concurrent MDS reviews to ensure achievement of maximum allowable PDPM categories.
  • Attend/participate in facility IDT meetings as directed.
  • Identify changes in condition in conjunction with IDT and coordinate scheduling and completion of indicated assessments.
  • Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.

Requirements:

  • Unencumbered State of CT RN or LPN License.
  • Experience with Point Click Care Preferred.
  • 1 year in Long Term Care.
  • Great attention to detail and great organizational skills.
