Responsibilities
  • Assure implementation of policies and procedures relating to Resident care.
  • Supervise and assist in provision of all Resident services, including coordination of those provided by independently contracted ancillary health care providers as requested by Residents and families.
  • Conduct assessments, negotiate service plans, develop service schedules and update service information as indicated by Resident needs.
  • Ensure that Residents are appropriately assessed on a timely basis and charged in accordance with those assessments.
  • Assist in Social History Assessments and outline life enrichment programming through these assessments, including meetings with residents and families and implementation and follow-up with associates and volunteers on life enrichment programming.
Requirements
  • Must be an LVN or LPN.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent problem-solving and organizational skills.
