Job Overview

ICAN Supported Employment Program

Position Summary:

The Case Manager plays a crucial role in the administration of client services within the Supported Employment Program. This position is dedicated to ensuring that clients achieve their individual service objectives and goals, aligning with the mission of ICAN. The ideal candidate will exhibit strong organizational skills, flexibility, attention to detail, and reliability.

Key Responsibilities:

  • Manage daily operations related to client services, ensuring effective support for individuals with developmental and physical disabilities.
  • Conduct the intake process for new clients, facilitating meetings with clients and support staff.
  • Prepare and maintain comprehensive reports and documentation for assigned caseloads.
  • Promote client autonomy and choice throughout the service delivery process.

Work Schedule:

This is a full-time, in-person role with a standard schedule of Monday through Friday, 8:30 AM to 5:00 PM.

Qualifications:

  • Minimum age of 21 years.

  • Experience in human services, particularly with individuals with disabilities, is preferred.

Education and Experience:

  • At least two years of college education in a relevant field or equivalent experience in human services.
  • Experience working with individuals with developmental disabilities is required.
  • Proficiency in computer applications, including Microsoft Office and client record databases.
  • Bilingual abilities, particularly in Spanish, are advantageous.

Professional Development:

  • Participation in ongoing professional development and training is mandatory, with a minimum of eight hours annually.

Essential Duties:

  • Review and reconcile client case notes daily to ensure accuracy and address any concerns.
  • Oversee the intake process for new clients, ensuring timely completion of necessary documentation.
  • Facilitate meetings to develop individualized plans that align with clients' goals and abilities.
  • Maintain accurate client records and ensure compliance with all relevant regulations and standards.

Team Collaboration:

  • Act as a liaison for clients, ensuring effective communication among all stakeholders.
  • Participate in team meetings and training sessions to enhance service delivery.
  • Monitor client progress and identify barriers to success, proposing solutions as needed.
