Director of Care Management Services (Full or Part-Time)

POSITION OVERVIEW (Full or Part-Time)

The Director of Care Management is accountable for the daily management of program services, supervising staff and interns, ensuring exceptional client service delivery, adherence to program grants and contracts, maintaining accurate fiscal and client records, and providing backup for invoice preparation and submission. Collaborating with the Vice President, the Director will identify, implement, and assess program goals and objectives by designing operational plans in accordance with each program's contractual and budgetary guidelines. This role includes clinical oversight of the Enhanced Care Management (ECM) program.

KEY RESPONSIBILITIES

  • Ensure clinical oversight and exceptional client service delivery.
  • Develop and sustain a viable ECM program. Prepare and supervise all essential ECM components, including client files, documentation, protocols, procedures, measurable deliverables, billing, and quarterly reports.
  • Exhibit strong clinical and diagnostic capabilities with expertise in bio-psycho-social assessments, care planning, and care coordination (addressing needs related to dental, medical, mental health, substance abuse, activities of daily living, instrumental activities of daily living, and housing situations).
  • Possess the ability to train and adapt assessment protocols in line with best practices and health plan regulations.
  • Review client case files for quality, timeliness, and accuracy; ensure all documentation (such as required assessments, forms, care plans, progress notes, closing letters, etc.) is complete and signed; ensure that internal and external program-specific and client-based portal systems align with hard copy client files and that files are maintained according to current clinical standards.
  • Serve as backup to monitor and ensure staff service recordings, including accurate billing, are entered promptly into designated software systems across all required programs.
  • Oversee and ensure program compliance with Wise & Healthy Aging policies and procedures; maintain proper documentation of all program services delivered to participants.
  • Act as the initial point of contact for referrals from various entities, such as city and county agencies and partnering healthcare organizations.
  • Screen potential Care Management and Peer Counseling clients. Oversee and assign clients/intakes to Care Coordinators, interns, and volunteers.
  • Evaluate that clients are being referred to the appropriate service modalities where they will receive various levels of case management (clinical and non-clinical) and access to essential benefits.
  • Provide one-on-one clinical supervision of Care Coordinators, interns, and volunteers as needed.
  • Facilitate biweekly staff meetings to review cases and coordinate training with staff, interns, and volunteers.
  • Provide direct client service when challenging situations arise that require supervisory support.
  • Deliver timely performance feedback formally (introductory reviews and annual performance evaluations) and informally to direct reports.
  • Assist in biannual and quarterly Care Management reporting.
  • Support the VP in coordinating and facilitating the completion and submission of requests for proposals to maintain current contracts and expand services as appropriate.
  • Serve as backup support, as needed, for the Vice President of Care Management.

ADDITIONAL RESPONSIBILITIES

  • Oversight of MSW year 1 and year 2 student interns.
  • Provide training, supervision, and problem-solving assistance to Care Coordinators on complex client cases.
  • Provide reporting and analytics to the Vice President, highlighting progress toward achieving set goals.
  • Other duties as assigned to accomplish program objectives.

POSITION REQUIREMENTS

Education

  • Minimum education requirement: A master's degree in social work, psychology, public health, or a related field. A licensed clinical social worker and licensed psychologist are preferred.

Experience

  • Experience supervising employees engaged in fieldwork.
  • Respond professionally to clients, constituents, and the broader community.
  • Prioritize and organize workload while responding promptly to clients and partnering entities or agency departments.
  • Demonstrate experience providing direct service in social services or a related field.
  • Prior management responsibilities involving case management duties.
  • Fieldwork experience with ethnically and economically diverse populations.
  • Bilingual language skills are a plus (e.g., Russian, Mandarin, Farsi, etc.).

  • Strong clinical, interpersonal, verbal, and written communication skills.
  • Ability to effectively communicate expectations to team members and demonstrate team building.
  • Ability to multitask effectively and prioritize work efficiently with competing deadlines.
  • Ability to demonstrate sound judgment and decision-making skills.
  • Proficient knowledge of Microsoft Office software programs.
  • Valid driver's license, auto insurance, and satisfactory criminal background check are required.
