Key Responsibilities of the MDS Coordinator:
- Manage case mix while striving to meet or exceed financial targets.
- Evaluate and assess residents for Medicare Part A eligibility prior to and following admission.
- Organize screenings and coverage determinations for prospective Medicare Part A candidates.
- Ensure compliance with Minimum Data Set (MDS) as per CMS regulations.
- Coordinate the scheduling of annual, quarterly, and significant change MDS assessments, ensuring timely submissions to relevant authorities.
- Conduct medical record reviews as mandated by CMS or HMO guidelines.
- Support nursing staff in maintaining MDS compliance.
- Provide training and updates to staff regarding MDS and care planning protocols.
- Develop and revise policies and procedures to ensure adherence to Medicare, Medicaid, and other insurance requirements.
- Monitor and assist in the timely completion of assessments.
- Oversee the input and transmission of MDS assessments to the state, confirming receipt.
- Facilitate care plan meetings and monthly quality management discussions.
- Revise and update patient care plans as necessary.
Qualifications for the MDS Coordinator:
- A Nursing Degree from an accredited institution is required.
- Must possess a current and active RN license in NYS.
- A minimum of 2 years of experience in Long Term Care as an MDS Coordinator is essential.
Essential Skills and Attributes of the MDS Coordinator:
- In-depth knowledge of Case-Mix, Federal Medicare PPS processes, and Medicaid reimbursement.
- Strong written and verbal communication skills, coupled with critical thinking capabilities.
- Comprehensive understanding of federal, state, and local regulations and standards.
- Ability to efficiently organize and prioritize tasks.
