Job Title: MDS Coordinator

Hurlbut Care Communities is seeking a skilled and experienced RN MDS Coordinator to join our team. As a key member of our nursing department, you will play a vital role in providing patient-centered care and ensuring the timely and accurate completion of the Minimum Data Set (MDS) and other related assessments.

Key Responsibilities:

  • Assess and evaluate residents for Medicare Part A coverage prior to and upon admission
  • Schedule and conduct screenings and determinations of coverage for all residents who may qualify for Medicare Part A
  • Complete and maintain MDS compliance per CMS guidelines
  • Coordinate scheduling for annual, quarterly, and significant change MDS and timely submission to government agencies
  • Perform medical record reviews as requested by CMS/HMOs using their guidelines
  • Oversee and assist nursing staff in maintaining MDS compliance
  • Train and update staff in MDS and care planning
  • Develop and update assigned policies and procedures to ensure nursing department compliance with Medicare, Medicaid, and other insurance reimbursement regulations
  • Monitor and guide the completion of assessments in a timely manner
  • Schedule and facilitate care plan conferences and quarterly meetings

Requirements:

  • Nursing degree from an accredited college or university
  • Current active RN license in NYS
  • At least 2 years' experience in Long Term Care as an MDS Coordinator
