The RN MDS Coordinator is a key member of our healthcare team, responsible for ensuring the timely and accurate completion of MDS assessments that meet federal and state guidelines. This role requires a strong leader who can oversee MDS assessors, coordinate care plan meetings, and ensure accurate diagnoses are documented in the EMR. The ideal candidate will have a current NYS nursing license, meet Department of Health requirements, and be available to work Monday through Friday, 9-5.
Key Responsibilities:
* Ensure accurate and timely completion of MDS assessments
* Oversee MDS assessors and coordinate care plan meetings
* Ensure accurate diagnoses are documented in the EMR
* Meet state and federal guidelines for MDS compliance
Requirements:
* Registered Nurse with a current NYS nursing license
* Meet Department of Health requirements
* Availability to work Monday through Friday, 9-5
