About the Role:

The Care Coordinator RN will work collaboratively with patients, families, and healthcare teams to provide comprehensive care coordination and support. This role will involve assessing patient needs, developing and implementing individualized care plans, and facilitating communication between healthcare providers and community resources. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:

* Assess patient needs and develop individualized care plans
* Coordinate care with healthcare providers and community resources
* Facilitate communication between patients, families, and healthcare teams
* Provide education and support to patients and families
* Collaborate with healthcare teams to ensure seamless care transitions

Requirements:

* Current RN licensure in New York State
* BSN degree or higher
* Certification in Care Coordination and Care Transitions
* 3-5 years of experience in clinical or community resource settings
* Excellent communication and interpersonal skills
