As a Patient Care Coordinator, you will engage collaboratively with a multidisciplinary team to support high-risk patients. Your role will involve providing care and managing diseases for patients with chronic conditions or those requiring surgical care.

Your key responsibilities will include:
- Utilizing advanced technological tools to oversee patient populations.
- Conducting thorough assessments and ongoing evaluations of patients' progress towards their health goals, updating care plans as necessary.
- Educating patients and their families on managing specific health conditions, including preventive measures and health maintenance tasks.
- Performing comprehensive assessments that encompass medical, behavioral, social, and end-of-life needs for each patient.

Job Responsibilities:
- Identify patients in the specialty care practice who require ongoing care coordination for their specific conditions.

- Define the nature and duration of involvement needed from the specialty care team and the primary care team.

- Employ assessment skills and risk assessment tools to identify patients with current or potential care needs requiring coordination.

- Conduct targeted outreach to a defined group of high-risk patients (those with chronic illnesses, limited social support, frequent readmissions, etc.) to ensure timely and effective care delivery.

- Utilize technological resources (registries, patient lists, care team tabs, etc.) to manage patient populations.

- Perform comprehensive clinical assessments that include disease-specific, age-specific, medical, behavioral, social, and end-of-life needs, sharing this information with the healthcare team.

- Collaborate with the interdisciplinary team to establish goals and interventions aimed at maximizing patient outcomes.
- Monitor patient adherence to the care plan.
- Conduct reassessments regarding patient progress towards goals and adjust care plans as needed.
- Ensure that care gaps are addressed for specialty diseases and chronic conditions.
- Act as the primary contact for patients regarding their conditions and facilitate access to necessary services.
- Coordinate the efforts of the patient care team.

- Serve as a liaison between patients, families, and healthcare providers, advocating for patients and addressing their concerns.

- Assist in managing transitions of care across different settings, ensuring effective communication and planning. Identify barriers to care and facilitate solutions.

- Collaborate with other care coordination teams, including primary care, transitional care, social work, rehabilitation, and pharmacy.

- Define and ensure adherence to disease-specific care pathways for specialty or chronic conditions.

- Work with patients and families to assess their knowledge, health literacy, and readiness to change, utilizing teach-back methods to evaluate understanding.

- Guide patients and families in self-management support, including setting both short- and long-term health goals.

- Provide education on managing specialty or surgical conditions, including preoperative, perioperative, postoperative, and recovery phases, along with preventive and health maintenance tasks.

- Connect patients with other care providers and community resources to enhance their care experience.
- Collaborate with practices on quality improvement initiatives.
- Perform other duties as assigned.

- Adhere to hospital and departmental policies and procedures, with particular attention to attendance, confidentiality, dress code, and completion of mandatory training to maintain competencies.


Education:
- Graduate from an accredited nursing program.

Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Current state licensure as a Registered Nurse (RN).
- Specialty certification is preferred.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).

All registered nurses in oncology settings must obtain a cancer-specific certification or demonstrate ongoing qualifying education within the accreditation cycle of the facility.
Complexity of Work:
- Requires critical thinking skills, decisive judgment, and the ability to work independently.
- Must be able to operate effectively in a high-pressure environment and take appropriate actions.

Work Experience:
- Three to five years of nursing experience is required.

Physical Requirements:
- Requires full range of motion, manual dexterity, and eye-hand coordination.
- Requires corrected hearing and vision within normal ranges.
- May involve exposure to communicable diseases or bodily fluids.

- Must be able to exert up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
