Role Summary

The Medical Social Worker is a vital member of Oak Street Health's care team, providing expert psychosocial support to patients and their caregivers. This role involves conducting comprehensive assessments, developing personalized care plans, and coordinating services to address the unique needs of our patients.

Main Responsibilities

  • Collaborate with interdisciplinary teams to identify patient needs and develop effective care strategies.
  • Provide ongoing support and education to patients and their families, promoting self-management and empowerment.
  • Connect patients with community resources and services, ensuring they receive holistic care.
  • Facilitate care coordination and communication among healthcare providers, ensuring seamless transitions and optimal patient outcomes.
  • Maintain accurate and timely documentation, ensuring all team members have visibility into patient progress and needs.

Requirements

  • Master of Social Work degree from a CSWE-accredited program.
  • Current licensure in the state where the OSH Center is located.
  • Fluency in Spanish, Polish, Russian, or other languages spoken by patients in our communities (where necessary).
  • Proficient computer skills, including Microsoft, GSuite, and electronic medical records.
  • US work authorization.

Preferred Qualifications

  • Experience in complex care coordination and population healthcare management.
  • Familiarity with using metrics and data to guide practice and drive outcomes.
  • Minimum one year of medical social work experience in a healthcare environment, preferably within the last two years.
