About the Role

As a Clinical Care Coordinator, you will be responsible for coordinating patient care, ensuring seamless transitions between healthcare settings, and promoting optimal health outcomes. Your expertise will be essential in developing and implementing individualized care plans, collaborating with multidisciplinary teams, and communicating effectively with patients and families.

Key Responsibilities
  • Coordinate patient care, ensuring timely and effective transitions between healthcare settings
  • Develop and implement individualized care plans, incorporating patient preferences and goals
  • Collaborate with multidisciplinary teams, including physicians, nurses, therapists, and social workers
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Monitor patient progress, identifying and addressing potential barriers to care
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices
Requirements
  • Bachelor's degree in Nursing (BSN) or related field
  • At least 2 years of experience in clinical home care or a related field
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Knowledge of state and federal home health agency regulations and compliance standards
  • Ability to work independently and as part of a team
