Job Summary

USA Health is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in providing compassionate care and support to our patients and their families.

Key Responsibilities
  • Conduct thorough assessments of patients' social, cultural, psychological, physical, and financial needs.
  • Develop and implement individualized discharge plans in collaboration with patients, families, and healthcare team members.
  • Provide crisis intervention and referral services to patients and families as needed.
  • Collaborate with Care Coordinators and Utilization Review nurses to coordinate and implement discharge plans.
  • Participate in interdisciplinary patient care rounds and conferences to ensure continuity of care.
Requirements
  • Master's degree in social work from an accredited institution.
  • Licensure with the State of Alabama as a Social Worker.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
