Job Description

The Nurse Care Coordinator will be responsible for coordinating patient care, developing individualized care plans, and implementing nursing interventions to achieve optimal patient outcomes. Key responsibilities include:




  • Assessing patients' needs and developing comprehensive care plans
  • Coordinating with healthcare teams to ensure seamless patient care
  • Providing patient education and support to promote healthy lifestyle choices
  • Communicating effectively with patients, families, and healthcare teams
  • Managing patient records and documentation
  • Participating in performance improvement activities and quality initiatives
  • Collaborating with multidisciplinary teams to achieve patient-centered care


Requirements

To be successful in this role, you will need:




  • A current Alabama RN license or a current Multi-State RN License in accordance with the Nurse Licensure Compact (NLC) for Alabama
  • A minimum of 2 years of Special Care Nursery experience
  • Excellent communication, interpersonal, and problem-solving skills
  • A strong commitment to delivering high-quality patient care and achieving patient-centered outcomes
  • The ability to work effectively in a fast-paced, dynamic environment
  • A willingness to participate in ongoing education and professional development
