Nurse Care Specialist

We are seeking a compassionate and skilled Registered Nurse to join our Home Health team at LHC Group. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities:- Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
