About the Role

We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional/Case Worker to join our team at Southeastern Integrated Care LLC. As a key member of our Intensive In-Home Team, you will play a vital role in providing high-quality, individualized care to children and adolescents with serious emotional, behavioral, and/or substance use disorders.

Key Responsibilities
  • Provide Intensive In-Home Services, a team approach designed to address the identified needs of children and adolescents.
  • Assist individuals with coping and crisis management, ensuring linkage to needed community services and resources.
  • Provide self-help and living skills training, parenting and independent living skills to help consumers and families build skills for coping with identified disorders.
  • Monitor and manage presenting psychiatric and addiction symptoms, working with caregivers to implement home-based behavioral supports.
  • Participate in treatment team planning, assist with PCP development, and follow-up on progress/lack of progress in meeting treatment goals.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a Human Service field with 2 years of full-time, post-bachelors degree accumulated MH/DD/SAS experience, or equivalent.
  • Valid driver's license and personal vehicle insurance coverage.
  • Ability to deliver services in various environments, including residential, school, workplace, and community settings.
