Position Overview

JOB TITLE: Admissions Nurse

REPORTS TO: Director of Nursing

This role is essential in ensuring that patients receive the compassionate care they deserve during challenging times.

Compensation: The annual salary for this position ranges from $80,000 to $100,000, based on education, experience, and relevant certifications.

Qualifications:

  • Must be a registered professional nurse, licensed to practice in New York State.
  • One to two years of recent clinical nursing experience with strong bedside skills.
  • A minimum of two years of recent experience in Medical-Surgical nursing.
  • A Bachelor's degree is highly preferred; a BSN is advantageous.
  • Experience in caring for terminally ill patients and a solid understanding of hospice principles.
  • Preferred experience in home care, hospice, and/or oncology settings.
  • Ability to connect with a diverse patient population and work effectively within a team.
  • Proficient in assessing the needs of terminally ill patients and their families, with the capability to develop care plans accordingly.
  • Basic knowledge of medications, treatments, and therapies for pain and symptom management.
  • Excellent oral and written communication skills, with strong listening abilities.
  • Familiarity with Microsoft Windows and experience with EMR systems.
  • Must possess a valid driver's license, vehicle access, and current insurance.
  • Active participation in coordinating all aspects of patient care in accordance with professional standards.
  • Ability to implement, evaluate, and provide direct care based on established patient care plans.
  • Competence in executing various treatments, procedures, and assessments.

Key Responsibilities:

  • Conduct initial meetings with patients and families at home or in healthcare facilities to explain services and assess needs.
  • Provide counseling and education to patients and families, detailing needs during the initial visit and clarifying the organization's philosophy and services.
  • Collaborate with hospital discharge planners to ensure timely and coordinated patient transitions from inpatient facilities.
  • Perform comprehensive assessments of patients and families.
  • Manage the admissions process, including:
    • Collecting demographic and medical information, documenting all necessary details.
    • Conducting physical and pain assessments, obtaining necessary consents, and preparing initial nursing and care plans.
    • Securing initial medical orders from the primary physician and notifying them of the admission.
    • Assisting the Intake Department with referrals and data entry as needed.
    • Supporting additional admission procedures as required.
    • Participating in weekend rotations as necessary.
    • Accepting other responsibilities as assigned.
