Key Responsibilities:

  • The Admissions Nurse (RN) will:
  • Conduct comprehensive physical and psychosocial evaluations of prospective hospice patients to create tailored care plans. These assessments may occur in various settings, including private residences and healthcare facilities.
  • Facilitate the admission of qualified patients into the hospice program, adhering to the established standards of practice set forth by relevant healthcare organizations.
  • Undertake additional responsibilities as required.

Required Qualifications:

  • A minimum of 2 years of experience in medical-surgical nursing, community health, oncology, or hospice care.
  • Graduation from an accredited nursing program.
  • Possession of a valid Registered Nurse (RN) license.

Preferred Skills:

  • Bachelor of Science in Nursing (BSN).
  • Prior experience in hospice care.
  • Experience in patient intake or admissions processes.
