As an integral part of our team, the Admissions Nurse plays a crucial role in engaging with patients referred for admission. This position involves informing patients, caregivers, and family members about the hospice services we offer, while also assessing patient eligibility for our programs.

Key Responsibilities:

  • Conduct visits to patients who have been referred for hospice services.
  • Communicate patient status effectively with the interdisciplinary team.
  • Foster strong relationships with hospitals, physicians, nursing facilities, and other referral sources through outreach and education.
  • Provide essential education to patients and caregivers regarding hospice care, and collaborate with the interdisciplinary team to create an initial care plan based on hospice and palliative care principles.
  • Generate and respond to referrals from hospitals and community partners.

Compensation:
The position offers a competitive rate of $68.71 per hour.

Qualifications:
Applicants must possess a current nursing license valid in the state of service, along with a valid driver's license and access to a registered and insured vehicle. CPR certification is also required.

Education:
Candidates should be graduates of an accredited nursing program, with a preference for those holding a BSN.

COVID-19 vaccination is strongly encouraged for all new hires prior to employment commencement at Calvary Hospital.
