Position Overview

JOB TITLE: Admissions Nurse

REPORTS TO: Director of Nursing

Are you passionate about making a significant impact in the lives of patients and their families? As an Admissions Nurse, you will utilize your exceptional assessment and communication abilities to assist individuals during critical moments in their healthcare journey. This role is essential in ensuring that patients receive the compassionate care they need when it matters most.

Compensation: The salary for this position ranges from $80,000 to $100,000 annually, based on education, experience, and relevant certifications.

Qualifications:

  • Must be a Registered Nurse, licensed to practice in New York State.
  • One to two years of recent clinical nursing experience with strong skills.
  • At least two years of recent experience in Medical-Surgical nursing.
  • A Bachelor's degree is preferred; a BSN is advantageous.
  • Experience with terminally ill patients and a commitment to hospice care is essential.
  • Preferred experience in home care, hospice, or oncology settings.
  • Ability to engage effectively with a diverse patient population.
  • Skilled in assessing the needs of terminally ill patients and their families.
  • Basic knowledge of medications and therapies for pain management.
  • Excellent verbal and written communication skills; strong listening abilities.
  • Team-oriented with the ability to collaborate effectively.
  • Proficient in Microsoft Windows and familiar with EMR systems.
  • Must possess a valid driver's license, vehicle access, and current insurance.
  • Actively involved in coordinating comprehensive patient care in line with professional standards.
  • Ability to develop and implement patient care plans and delegate tasks as necessary.
  • Capable of performing various treatments and assessments.

Key Responsibilities:

  • Conduct initial meetings with patients and families to explain services and assess needs.
  • Provide counseling and education to patients and families regarding care options and advance directives.
  • Collaborate with hospital discharge planners for seamless patient transitions from inpatient facilities.
  • Perform comprehensive assessments of patients and families.
  • Manage admissions processes, including:
    • Collecting demographic and medical information and documenting it accurately.
    • Conducting physical and pain assessments, obtaining necessary consents, and preparing initial care plans.
    • Securing initial medical orders from primary physicians and notifying them of admissions.
    • Assisting the Intake Department with referrals and data entry as needed.
    • Participating in weekend rotations as required.
    • Taking on additional responsibilities as assigned.
