About the Role

We are seeking a skilled Health Services Coordinator to join our team at Bridgewell. This individual will be responsible for providing health services and direct care to individuals with disabilities living in a residential program.

Key Responsibilities:

  • Assess and organize health care needs for assigned individuals, including medication management and coordination with community healthcare providers.
  • Collaborate with registered nurses and other healthcare professionals to ensure comprehensive patient care.
  • Develop and maintain accurate and up-to-date medical records, adhering to relevant laws and regulations.
