CareSTL Health is looking for a dedicated Community Health Worker to join our Population Health Department. The Community Health Worker will play a crucial role in educating and connecting patients, families, and communities with essential resources by collaborating with local agencies.
Key Responsibilities:
- Conduct outreach initiatives to engage high-risk individuals and those not currently receiving care.
- Utilize the Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) tool to assess patient needs and address barriers, providing necessary resources.
- Encourage patient involvement in their healthcare, including preventive measures, chronic disease management, and self-care strategies.
- Support the primary care team in enhancing care management processes through guidelines, disease management techniques, and patient education to foster better self-management of chronic conditions.
- Maintain follow-up with patients through various communication methods, including phone calls, Telehealth, home visits, and other accessible settings.
- Empower and coach patients while acting as a liaison between them, the clinical care team, and community partners.
- Assist patients in addressing their social determinants of health.
- Facilitate access to community-based services.
- Assess crisis situations and implement appropriate interventions.
- Collaborate with team members to enhance prevention strategies and develop solutions to overcome healthcare access barriers.
- Connect patients with transportation resources and provide appointment reminders when necessary, while adhering to policies regarding transportation.
- Engage in general support activities and special projects as needed.
- Familiarity with harm reduction strategies, motivational interviewing, and relevant treatment practices.
- Understanding of local community culture and values.
- Knowledge of customer service principles, including needs assessment and quality service standards.
- Strong listening skills and the ability to comprehend and engage with others effectively.
- Clear communication skills, both verbal and written.
- Proficiency in Microsoft Excel.
- High School Diploma or GED Equivalent required.
- Completion of a Community Health Worker (CHW) training program is mandatory.
- Current Community Health Worker Certification is required.
- A minimum of 2 years of experience in community health, human services, outreach, or healthcare settings.
- Experience in customer service, patient coaching, medical interactions, community relations, outreach, data entry, public speaking, and non-profit work is advantageous.
- Must possess a valid driver's license and have reliable transportation.
- Clinic operates Monday to Friday from 8 a.m. to 5 p.m.
- Offers 11 paid holidays annually.
