Job Summary:

The Outreach Specialist at People's Family of Corporations provides intensive outreach and engagement to youth and their caregivers, responding to crisis situations and connecting them with necessary services.

Key Responsibilities:

  • Collaborate with the ERE Team to assist clients in accessing psychiatric, medical, and social services.
  • Network with community providers to deliver a comprehensive safety-net for individuals served.
  • Develop recovery-oriented care plans addressing behavioral health, medical, and social barriers.
  • Build and maintain knowledge of community resources to match clients with eligible services.
  • Participate in weekly ERE Outreach Team meetings and ongoing training opportunities.

Requirements:

  • Bachelor's Degree in a social services field, Master's degree preferred.
  • Minimum of 2 years' experience providing services to youth with severe emotional disturbance.
  • Knowledge of the Eastern Region Mental Health Network, community resources, and the DSM-5.
  • Ability to work with persons with multiple challenges and build healthy partnerships with external providers.
