About the Role
We are hiring a Registered Nurse Case Manager to join our team of skilled caregivers. The successful candidate will have a passion for providing high-quality care to patients with terminal illnesses and their families. Responsibilities
- Screen referrals for appropriateness and perform admissions to facilitate the transition of patients and families into the hospice program
- Case manage patients and families to ensure symptom management, patient and family education, medication management, coordination of hospice services, and excellent communication with the patient's physician
- Coordinate the interdisciplinary care team for assigned patients and implement the plan of care
- Provide primary nursing care for hospice patients, regardless of the setting
- Teach primary caregivers skills needed to care for patients in their residences
- Complete nursing assessments and admissions of patients
- Maintain clinical records to meet federal, state, and agency regulations and guidelines
- Supervise hospice aides and certified nurse aides

Required Skills and Qualifications
- A thorough understanding of hospice regulations (federal, state, and local government)
- Ability to handle confidential matters
- Ability to work a flexible schedule
- Commitment to the hospice concept and standards
- Participate in agency surveys and required reports
- Attend continuing educational programs to keep up-to-date with changes in the field and maintain current license/certification, as required
