Job Description:
We are seeking a skilled Nurse Practitioner to join our team. As a Nurse Practitioner at Abode Care Partners, you will be responsible for providing evidence-based medical care to our patients. This includes examining and treating chronic illnesses, ordering diagnostic testing, and collaborating with healthcare professionals to maintain comprehensive care plans.
Responsibilities:
- Provide primary and specialty care to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.
- Conduct physical exams, diagnose illnesses, and develop treatment plans tailored to each patient's needs.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive care plans.
- Provide referrals to other healthcare practitioners when necessary.
- Ensure patient medical records are kept up to date.
Required Skills and Qualifications:
- Licensure: Active state Nurse Practitioners license
- Certification: Current Nurse Practitioner National Board Certification
- Education: Master's degree in Nursing (MSN)
- Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, Strong knowledge base of HIPAA and OSHA regulations, Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
