Job Summary: We are seeking an experienced Hospice & Palliative Care Nurse Practitioner to provide high-quality care to our patients on a per diem basis.

- The ideal candidate will have excellent communication and organizational skills, as well as experience working in hospice or palliative care nursing.
Responsibilities:
- Implement palliative care program requirements
- Conduct face-to-face encounters with patients
- Communicate effectively with patients, families, and agency personnel
