**Job Description:**

We are seeking an experienced End-of-Life Care Specialist to join our team. As a registered nurse with a passion for hospice care, you will be responsible for providing comprehensive care to patients in their homes, focusing on symptom management, pain control, and emotional support.

**Key Responsibilities:

  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their loved ones on end-of-life care, including symptom management, medication management, and advanced care planning.
  • Maintain accurate and timely documentation of patient interactions, care plans, and outcomes.

**Requirements:**

  • Current RN licensure in the state of practice.
  • 2+ years of clinical nursing experience, with at least 1 year of hospice nursing experience preferred.
  • Knowledge of reimbursement guidelines and regulatory requirements.
  • CPR certification and continuing education requirements.
