Patient Access Coordinator

The Patient Access Coordinator is a critical role within the Veterans Health Administration, responsible for ensuring seamless coordination of services for hospitalized Veterans. This position requires a skilled nurse with excellent communication and organizational skills to facilitate effective care delivery.
Key Responsibilities- Coordinate care for Veterans hospitalized in local community non-VA and DOD hospitals, documenting results and following up until discharge.
- Manage and facilitate transfers into VAPSHCS, ensuring timely and efficient care delivery.
- Provide alerts and notifications for other VA staff, including providers and social workers, to ensure a holistic approach to care coordination.
- Work closely with community care managers, social workers, providers, and nurses to assist in discharge planning and follow-up care.
- Coordinate care for Veterans traveling to and/or from other VAs who require services from VAPSHCS, including within our VISN or region, and interfacility transfer coordination for VA to VA for acute care.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).

- Care Coordination/Case Management experience
Monday - Friday; 7:30am - 4pm or 8am - 4:30pm
