Job Summary

The Referral Coordinator position at Seattle Indian Health Board is responsible for ensuring patients have access to specialty care services by developing exceptional relationships with providers, relatives, and community partners to ensure specialty care needs are met.

Key Responsibilities
  • Track referral statuses and coordinate specialty care for patients
  • Work with care coordinators and the care team to ensure an exceptional patient experience
  • Develop and maintain relationships with SIHB providers, relatives, and community partners
  • Assist patients and patient-services staff with primary care physician (PCP) changes to facilitate referrals
  • Document referral outcomes for providers using organization's software
  • Follow up with ordering providers and collect and route specialists' chart notes to the appropriate provider
  • Complete patient-records requests
  • Collaborate with Care Coordinators to assist relatives in obtaining medical care and scheduling appointments
Requirements
  • 1+ year(s) of clinical work with patient care/case management
  • 1+ year(s) of customer service experience
  • EPIC experience highly desirable
  • Strong physical assessment skills
  • Strong organizational, time management and interpersonal skills
  • Ability to multi-task comfortably while working with a high degree of professionalism
  • Exceptional interpersonal skills with the ability to develop relationships with Relatives and provide them with a positive experience
  • Proficiency in Microsoft Suite programs
  • Ability to demonstrate strong written, verbal and interpersonal communication skills with varying levels of internal and external agencies and medical professionals
Preferred Qualifications
  • 2+ years using EPIC
Work Environment

The Referral Coordinator will work in a normal office environment with consistent interpersonal engagement with patients and staff. Standard hours are 7 am to 6 pm, 4 days a week, with occasional extra hours for events or to meet deadlines.
