About the Role

We are seeking a highly skilled Registered Nurse to join our team as an MDS Coordinator. In this role, you will be responsible for scheduling MDS and care plans according to regulations, completing the MDS and care plan timely and accurately, and participating in staff education relative to regulations of the RAI process.

Key Responsibilities
  • Schedule MDS and care plans according to rules and regulations
  • Complete the MDS and care plan timely and accurately according to regulations
  • Conduct resident care plan meetings, updating care plans to meet regulations and policy
  • Participate in staff education relative to regulations of the RAI process
  • Submit appropriate MDS/RAI data to the federal database
  • Facilitate effective communication between MDS and other departments
Requirements
  • Associate degree in Nursing from an accredited college
  • License to practice nursing in the State of Indiana as a Registered Nurse
  • Must hold and maintain a CPR certification
  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records
  • Good interpersonal skills, good verbal, and written communication skills
  • Ability to understand and apply medical and facility terminology
  • Ability to use MS Office applications and electronic patient records
  • Ability to serve as a patient advocate
  • Knowledge of and the ability to interpret nursing and facility policies and procedures
