Job Summary

We are seeking a skilled Care Manager III to join our team. As a Care Manager III, you will work collaboratively with our healthcare team to coordinate patient care, assess patient needs, and develop effective care plans. Your expertise will ensure that our patients receive comprehensive and compassionate care, leading to improved health outcomes and patient satisfaction.

Key Responsibilities

  • Collaborate with healthcare team members to coordinate patient care and develop effective care plans.
  • Assess patient needs and develop strategies to meet those needs.
  • Connect patients with relevant community resources to improve health outcomes and patient satisfaction.
  • Delegate responsibilities to staff members to ensure efficient and effective care delivery.
  • Implement discharge plans in collaboration with patients, families, and physicians.
  • Provide education and support to patients, families, and healthcare team members.
  • Act as a liaison between patients, physicians, and outside agencies to coordinate care and exchange information.
  • Maintain accurate and up-to-date patient records.

Qualifications

  • Associate's Degree or higher in a related field.
  • Registered Nurse (RN) licensure.
  • Basic Life Support (BLS) certification.
  • At least 5 years of experience in nursing or patient care.
  • 1 year of experience in case management preferred.

Travel Requirements

Up to 20% travel may be required for this role.
