Job Summary
We are seeking a skilled Care Manager III to join our team. As a key member of our healthcare team, you will work collaboratively with patients, families, and healthcare professionals to coordinate patient care and ensure seamless transitions.
Key Responsibilities
- Conduct comprehensive patient assessments and develop personalized care plans to meet patient needs.
- Collaborate with healthcare teams to coordinate patient care, including medication management, treatment plans, and discharge planning.
- Connect patients with community resources and services to promote health, well-being, and patient satisfaction.
- Provide education and support to patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
- Act as a liaison between patients, healthcare providers, and external agencies to facilitate communication and coordination.
- Maintain accurate and up-to-date patient records, including medical history, treatment plans, and progress notes.
Qualifications
- Associate's Degree in Nursing or related field required; Bachelor's Degree preferred.
- Registered Nurse (RN) licensure required; Certified Case Manager (CCM) certification preferred.
- Basic Life Support (BLS) certification required; 5 years of nursing or patient care experience required; 1 year of case management experience preferred.
Travel Requirements
Up to 20% travel may be required for this role.
