- Act as a liaison between patients, healthcare providers, and other stakeholders to facilitate communication and resolve issues.
- Maintain accurate and up-to-date patient records and reports to ensure compliance with regulatory requirements.
- Associate's degree in a related field (e.g., nursing, healthcare administration)
- Registered Nurse (RN) licensure
- Basic Life Support (BLS) certification
- Minimum 5 years of experience in patient care or case management
- Preferred qualifications: Bachelor's degree, Certified Case Manager (CCM) certification, and 1 year of case management experience
