We are currently seeking a full-time Intake Coordinator for our operations. This position operates Monday through Friday from 8 AM to 5 PM, with a weekend rotation. Training will be conducted entirely on-site, with potential for partial remote work following successful training.

Position Summary:

This entry-level role is crucial for the comprehensive coordination of care for patients receiving home infusion therapy. A Senior Intake Coordinator will serve as a mentor to guide the new hire.

The Intake Coordinator position offers a structured Career Ladder. Candidates demonstrating outstanding performance, commitment to personal growth, and strong collaboration within the department may be eligible for advancement to:

  • Senior Intake Coordinator: Promotion is possible within 2-5 years for those meeting specific goals and showing active engagement in departmental activities.
  • Lead Intake Coordinator: This role involves leadership responsibilities and engagement with departmental and organizational initiatives.

Key Responsibilities:

  • Compile initial Patient Profiles for therapy and distribute to the healthcare team.
  • Manage incoming calls for the Intake Workgroup, ensuring proper documentation.
  • Communicate effectively with pharmacy and delivery staff regarding patient referrals and issues.
  • Collaborate with the Liaison/Care Coordination team to facilitate efficient patient discharge.
  • Document all patient-related information in clinical records accurately.
  • Update patient supply requirements and communicate changes to relevant teams.
  • Process delivery requests following established procedures.
  • Correct any errors in patient files as identified through daily reports.
  • Adhere to company policies and procedures during daily operations.
  • Develop understanding of insurance coverage with guidance from senior staff.
