Job Summary

We are seeking a highly skilled Clinical Admission Nurse to join our team at Northwestern Medicine. The successful candidate will be responsible for coordinating patient admissions, ensuring a smooth transition from hospital to home, and promoting efficient patient flow.

Key Responsibilities
  • Access Point Resource: Serve as a primary point of contact for physicians, staff, and patients, providing timely and accurate information to facilitate patient admissions.
  • Admission Coordination: Collaborate with healthcare teams to ensure patients receive the most appropriate level of care, based on their medical needs and insurance coverage.
  • Clinical Decision Making: Apply approved criteria and guidelines to make informed decisions about patient admissions, taking into account managed care rules and practices.
  • Communication and Coordination: Effectively communicate with patients, families, and healthcare providers to ensure seamless transitions and optimal patient outcomes.
  • Transportation and Logistics: Arrange transportation for patients as needed, coordinate services, and notify points of entry of patient arrivals.
  • Medical Necessity: Query admitting physicians for additional information to ensure medical necessity for admission or appropriate level of care.
  • Outlier Cases: Refer cases to physician advisors for review and guidance on outlier cases.
  • Registration and Demographics: Coordinate admission with registration staff to obtain accurate patient demographics.
  • Referrals and Coordination: Refer patients to patient financial services, risk management, peer review, infection control, and patient relations as needed.
  • Outpatient Resources: Knowledgeable of available outpatient resources and sites of care within the organization for referral to lesser levels of care when appropriate.
  • Patient Flow and Throughput: Coordinate patient activity and flow or movement between levels of care, identifying potential throughput barriers and proactively informing upper management of anticipated delays.
  • Clinical Review and Determination: Review clinical information against Milliman criteria to determine patient class and level of care needed.
  • MD Consultation: Confer with physicians to discuss clinical and appropriate level of care when needed.
Qualifications
  • Education: Bachelor of Science in Nursing (BSN) required for all new hires.
  • Experience: At least two to five years of patient care experience in a hospital setting.
  • Critical Thinking and Problem-Solving: Able to multi-task quickly, process multi-factorial stimulation accurately, and make decisive judgments with minimal supervision.
  • Preferred Qualifications: Past experience in admissions, utilization review activities, discharge planning, and/or case management.
