We are seeking a highly skilled Clinical Admission Nurse to join our team at Northwestern Medicine. The successful candidate will be responsible for coordinating clinical needs with operational processes and insurance benefits to ensure a smooth admission experience for patients.
Key Responsibilities- Access Point Resource: Serve as a resource for physicians, staff, and patients, providing timely and accurate information to facilitate the admission process.
- Admission Coordination: Assist with determining the most appropriate level of care and admission status for patients, applying approved criteria and guidelines in decision-making.
- Clinical Decision-Making: Apply national guidelines and clear communication with physicians to ensure accurate and efficient admission decisions.
- Transportation and Coordination: Arrange transportation for patients as needed, coordinate services, and notify points of entry of patient's impending arrival.
- Medical Necessity: Query the admitting physician for additional information as needed to meet medical necessity for admission or appropriate level of care.
- Outlier Cases: Refer cases to physician advisor for any outlier cases.
- Registration and Demographics: Coordinate admission with registration staff to obtain accurate patient demographics as indicated.
- Referrals and Coordination: Refer appropriate cases to patient financial services, risk, peer review, infection control, and patient relations.
- Outpatient Resources: Knowledgeable of available outpatient resources and sites of care within the organization for referral to lesser levels of care when appropriate.
- Patient Flow and Movement: Coordinate patient activity and flow or movement between levels of care.
- Throughput Barriers: Identify potential throughput barriers and act proactively to inform and alert upper management of anticipated delays.
- Clinical Review: Review clinical against Milliman criteria to help determine patient class and level of care needed.
- MD Consultation: Confers with MD to discuss clinical and appropriate level of care when needed.
- Education: BSN required for all new hires as of 1/1/2011.
- Experience: At least two to five years of patient care experience in a hospital setting.
- Critical Thinking and Decision-Making: Able to multi-task quickly, process multi-factorial stimulation accurately, and requires critical thinking skills, decisive judgment, and the ability to work with minimum supervision.
- Preferred Qualifications: Past experience in admissions, utilization review activities, discharge planning, and/or case management.
