As a key member of our interdisciplinary team, you will provide high-quality nursing care to pregnant and postpartum women, promoting health and well-being for themselves and their infants.
Key Responsibilities- Assess and prioritize client needs, providing education and support to promote self-efficacy and healthy decision-making.
- Collaborate with the MSS team and other healthcare providers to ensure comprehensive care.
- Document all nursing visits and interventions in the client's electronic health record.
- Participate in monthly provider meetings and quarterly Community First Steps meetings.
- BSN/RN with recent maternal/infant, community, or public health experience.
- Effective communication and interpersonal skills, with ability to work with diverse populations.
- Basic Life Support (BLS) CPR certification required within 90 days of hire.
- Access to an insured vehicle and ability to make home visits.
